At any point from when it is offered to when it is put in front of a judge.

Up until the judge accepts the terms of the agreement. Most people don't realize that even if the prosecutor and the defense agree on a plea bargain agreement, the judge still can choose not to accept the agreement if she feels it is not in the interest of justice. Prior to the judge accepting the agreement, either party can back out at any time and for any reason (or no reason at all).
